About the job: We are seeking an Agency Case Manager to coordinate service delivery for individuals served in the agency. You will act as the primary contact for the supported individuals, their families/guardians, other agency providers, and funding sources. The successful Agency Case Manager will provide high quality case coordination between day and residential programming, develop active treatment programs / supports to ensure that individuals served reach their personal goals and achieve outcomes, analyze and develop individual program plans, prepare professional reports, coordinate and facilitate program planning meetings, and regularly visit supported individuals in day programming and residential locations. Schedule: The primary office hours for this salaried position are Monday-Friday from 8:00 a.m.- 4:30 p.m. Some early mornings, evenings and weekends are necessary as well as being on call.

Salary: $38,319.47 - $45,286.86

Required qualifications:

  • Bachelor’s degree in social work, special education, psychology, human services or a related field

  • 1-2 years’ relevant experience required

  • Behavioral experience preferred.

  • Must be able to promote, escort or physically restraint as needed.

  • Must be able to lift 20 lbs. regularly and up to 50 lbs occasionally

  • Excellent verbal and written communication skills.

  • Strong computer skills

  • Must possess a valid driver's license and provide proof of insurance.

  • Some travel is required
     

The successful candidate is required to successfully pass a background check and complete a pre-employment physical and drug screen as a condition of employment. In accordance with the Centers for Medicare and Medicaid Services mandate, candidates for employment must provide evidence of their first COVID-19 vaccine prior to the start of employment, or successfully request a medical or religious exemption to this requirement. The second vaccine is required within 30 days unless exempted.
